It's the ? in the middle of the compass, anywhere in the world. The 5 arrows point from it to real cities around it: an arrow pointing north to Oslo means the hidden city lies south of Oslo. Hard mode has less famous cities, and no clue city is closer than 500 km.

After your second wrong guess, each arrow shows how far away its city is. After the third, your guesses show how far they are from the hidden city. (In default mode, the hints come one guess earlier.)

Your score is all your guesses added up; a city not found counts as 6. Gold for up to 5, silver for up to 10, bronze for anything else with a city found. A new challenge starts every day at midnight (Berlin time), the same for everyone. Share your result: it doesn't give the cities away.
